We are limited to 20 students per clinic.
OSNS needs the below form filled out for each individual student.
The donation amount for the 2010 Clinics are $25 per student.
As of March 1, 2009, we ask that all monthly committed members donate HALF of the clinic cost.
Donations can now be paid with a credit card here online (below),
mailed to P.O. Box 940642, Plano, TX 75094, or you can bring donations to the gym.
All checks need to have the name of the student and "clinic" in the memo. If you donate with cash
please put in an envelope with students name and "clinic" on the envelope.
**If funds are not specified, we will consider it as a love offering donation!**
